• Comprehensive Assessment: A thorough evaluation of the patient's physical, emotional, social, and spiritual needs related to their illness. This includes:
  
  • Detailed medical history review
  • Symptom assessment (pain, nausea, fatigue, anxiety, depression, etc.)
  • Functional status assessment
  • Psychosocial assessment
  • Spiritual assessment
• Symptom Management: Development and implementation of a personalized plan of care to manage distressing symptoms, which may include:
  
  • Medication management
  • Non-pharmacological interventions (e.g., relaxation techniques, massage, guided imagery)
  • Education on symptom management strategies
• Care Coordination: Collaboration with the patient's existing healthcare team (physicians, specialists, nurses, etc.) to ensure seamless and coordinated care. This includes:
  
  • Communication with primary care physicians and specialists
  • Collaboration with hospitals and other healthcare facilities
  • Referrals to other community resources as needed
• Advance Care Planning: Facilitation of discussions regarding goals of care, advance directives (e.g., living will, durable power of attorney for healthcare), and end-of-life wishes. This includes:
  
  • Education about advance care planning
  • Assistance in completing advance directives
  • Facilitation of family meetings to discuss goals of care

B. Psychosocial and Emotional Support:

• Counseling and Support: Providing emotional and psychosocial support to patients and their families, including:
  
  • Individual counseling
  • Family counseling
  • Support groups (if available)
  • Grief counseling
• Resource Navigation: Connecting patients and families with community resources, such as:
  
  • Financial assistance programs
  • Support organizations
  • Respite care services
  • Transportation assistance
• Spiritual Support: Addressing the spiritual needs of patients and families, which may include:
  
  • Facilitating connections with chaplains or spiritual advisors
  • Providing a safe space for exploring spiritual beliefs and values

• Pain and symptom management
• Improved quality of life
• Emotional and psychosocial support
• Enhanced communication between patient, family, and healthcare providers
• Clarification of goals of care and advance care planning
• Support for caregivers
• Coordination of care across healthcare settings

Our focus is on improving quality of life for patients and their families by addressing physical, emotional, social, and spiritual needs. We offer these services concurrently with curative treatment, unlike hospice care which focuses on end-of-life care. 

Patients eligible for non-hospice palliative care services at Indigo Sun typically include individuals with serious illnesses such as:

• Kidney disease
• Neurological disorders
